Paper and packaging giant Visy has been fined $375,000 in a New South Wales court after a workplace accident that ultimately saw a man's leg amputated.

On June 25, 2020, an employee working on a conveyor at the company's Smithfield worksite in Sydney unknowingly stepped into the path of a trolley car, according to a release from SafeWork NSW.

The car hit the man's left leg, trapping it against the conveyor and crushing his left ankle.

The man underwent multiple surgical procedures to his left ankle and foot.

Following surgery, his left leg was surgically amputated below the knee due to an infection.

SafeWork NSW said a risk assessment carried out at the Smithfield workplace about six months before the accident had raised concerns regarding "crush/pinch points between edge of trolley car and conveyors corners".

Solutions outlined in the assessment were not implemented, SafeWork NSW said.

"Visy were aware of the danger their employees were working in and, despite being given steps to eliminate or minimise the risk, did not act appropriately to address the risk," SafeWork NSW head Trent Curtin said.

"Mobile plant, including trolley cars, have the potential to seriously injure or kill people. Workplaces need to identify risks such as where mobile plant might interact with people and design their workplace layout so that vehicles and pedestrians are separate wherever possible."
